HRT — Frequently asked questions
HRT (hormone replacement therapy) replaces estrogen — and progesterone if uterus present — during menopause transition. Most common questions: timing, formulations, safety, side effects, cost.
13 questions answered by our NAMS-certified editorial team.
Can I take GLP-1 medications with HRT?
YesYes, GLP-1 medications (semaglutide, tirzepatide) and hormone therapy can generally be taken together with no known interactions. Many perimenopausal and postmenopausal women take both safely.
Why does perimenopause cause anxiety?
Fluctuating estrogen disrupts serotonin and norepinephrine — both involved in mood regulation. Anxiety in perimenopause is biological, not just "stress."
How long do hot flashes last?
Median total duration is 7.4 years per the SWAN study — longer than previously thought. About 1 in 3 women experience them for 10+ years.
When should I start HRT?
NAMS recommends HRT initiation under age 60 or within 10 years of menopause onset for maximum safety profile. Earlier within this window often preferable for symptom control.
Can I take HRT with thyroid medication?
YesYes, HRT and thyroid hormones (levothyroxine, liothyronine) generally combine safely. Oral estrogen can raise thyroid-binding globulin — may require thyroid dose adjustment.
Is compounded progesterone safe?
It dependsCompounded progesterone is safe when prescribed by a licensed clinician through 503A pharmacy. Most evidence supports FDA-approved oral micronized progesterone (Prometrium) over custom-compounded.
How long does it take for HRT to work?
Hot flashes improve in 2-4 weeks. Sleep + mood in 4-8 weeks. Vaginal symptoms in 8-12 weeks. Bone density takes 12+ months. Full benefit assessment at 3 months.
Can I get pregnant on HRT?
YesPregnancy is technically possible during perimenopause up to menopause confirmation (12 cycle-free months). HRT is NOT contraception. Continue contraception up to menopause confirmed.
Why does perimenopause cause weight gain?
Three factors: estrogen-mediated fat redistribution from hips to abdomen, ~1%/year muscle loss from sarcopenia, and 30-50% drop in insulin sensitivity. Same calorie intake = more weight.
How long does vaginal atrophy treatment take to work?
Topical estrogen improves dryness in 2-4 weeks, full epithelial restoration at 12 weeks. Ospemifene (oral) takes 8-12 weeks. DHEA suppositories work similarly.
When should I start tracking perimenopause symptoms?
Start tracking when cycle length shortens by 7+ days OR you notice new sleep/mood/VMS changes. Most useful between age 35-50. Bring 3-month log to clinician visit.
Why does menopause cause joint pain?
Estrogen has anti-inflammatory effects via cortisol modulation. Decline triggers low-grade systemic inflammation, manifesting as joint stiffness and pain — especially fingers, hands, knees.
How long should postmenopausal bleeding take to investigate?
Any postmenopausal bleeding (12+ months after last period) warrants evaluation within 1-2 weeks. Endometrial cancer accounts for ~10% of cases — urgent workup.
